Contest Rules

GIVEAWAY: Dr. Liberto is giving away an iPad to the patient with the most referrals starting April 1, 2012 through July 1, 2012. Check out the rules and be eligible to win.

DATES: The contest begins on April 1, 2012 and closes on June 30, 2012 (the contest "closing date")

HOW TO ENTER: New patients, aged 3 and older, must mention that the existing patient referred the new patient. Each time an existing patient is mentioned, this patient will receive one referral point for each new patient.

WINNER SELECTION: The patient with the most referral points on our Where do you Stand? page by the end of a three month period will win.

PRIZE: Prize includes an Apple iPad Wi-Fi 16GB giveaway.

  1. Each eligible winner will receive one Apple iPad. A total of 4 prizes will be awarded over the course of a year in this sweepstakes. The actual retail value (ARV) of each prize is $499.00 and the combined ARV of all four prizes is $2,000.00. Each winner is limited to one iPad and does not include service activation or accessories. Winner is responsible for all service activation pertaining to prize and any other fees, chrages, or equipment required to use the prize.
  2. Prize must be accepted as awarded and cannot be redeemed for cash or substituted for any other items by the winners.
  3. Winners must agree to give out his or her name and be photographed to be used for promotional purposes.
